repeating of controller in url

Hi i made form in codeigniter with validations! when i submit this first time it runs correct! but if i submit it again there will be page not found error! and in url controller repeat two times like :: first time url is:http://localhost/test/first/signup_check second time url is :http://localhost/test/first/first/signup_check what is this problem !
