Need help in making a multiple intelligence type online test

There are 8 intelligence types and my online test will consist of 5 statements for each of them. The user will then answer in total of 40 statements with "Never, rarely, often, always". Then after answering they will get a result based on the intelligence type that got the highest score.

I'm really new to coding and I have no experience in programming languages other than HTML and CSS (if they can even be considered as such), but I believe that I'm a fast learner. Sadly, I need to make an online test to pass a subject so I really need help.

I know I can put values to the choices (ex. Never(0), rarely(1), etc.,) but since I have no experience I really have no idea what to do anymore.
