Web-testing report formality - how to approach my first report?

As a task to get higher score from one of my uni subjects I got a task of creating a report for simple webpage test(catching dead links, bugs, testing inputs etc.) in any format preferable. I thought that I could kill two birds with one stone, and not do it in a least-effort way, but in more proffesional one. Maybe I will be able to include it in my resume for example.

Here come's my question - what's the way of presenting such report? Is there any "generally accepted" way of handling it?
