What is a good way to organize/document test cases?

How should one go about organizing tests that should be run, have been run, and their results? Previously a spreadsheet was used, however I would like to keep the test case information with the source code. Here are the fields the test case descriptions would have. Ideally I would like to keep it within the Issues tab or somewhere that makes sense. What are the conventions with documenting the testing of software?
