wordpress survey plugin that returns text

I am looking for a wordpress plugin form for a questionnaire that returns text.

For example, if I ask the following questions.

1. Do you like working outside. yes/no
2. DO you like working alone. yes/no
3. Do you like physical labor. yas/mo

Depending on the site visitors answers, they will receive text that outputs a preset paragraph that tells them what jobs they're best fit for.

What I want to do is a bit more complicated but this is pretty much it.
