how to do performance testing of a website?

how to do performance testing of a website and create a complete performance testing report. what we need to include in the report and what tools needed to do performance testing,load,stress testing etc. i have used jmeter and created some graphs for Response Time over Time, Hits per second, Bytes throughput over time, Response codes per second, Response Time Distribution, Response Time Percentile, Response Time vs Threads, Active Thread over time, Transactions per second , Transaction Throughput vs Thread
